Our new Electronic Medical Record is now up and running. You may now schedule your first appointment after registering your demographic and insurance information!
step 1: go to and select SIGN UP on the top tab
step 2. You will provide your e-mail address and choose an ID.  A temporary password will be e-mailed to you so you can now LOG IN. You will then need to create your own PASSWORD
step 3. Select MY PROVIDERS on the top menu and click SEARCH. You will find me in the list after selecting WA (state). Then click ADD.  You will also need to ender my SECURITY CODE 147862.
step 4: You can now go to PROFILE on the top tab and select PATIENT PROFILE to upload your Informations (Demographic, Medical History, Medication, Allergy, Preferred pharmacy and Insurance)
step 5: select HEALTH CARE REQUEST on the top tab and select APPOINTMENT to make your first appointment.
This site is encrypted and HIPPA compliant secure.
We hope to open our doors to see patient beginning March 28, 2010.
HINT: You can also go to the HELP menu to view various functions that are available.

Coronavirus Update

Protocol for David Lee's Office

    1. Policy begins March 16, 2020 for the COVID-19 virus

    2. Any patient with fever and respiratory symptoms such as cough or chest congestion will need to call the office first and speak to Dr. Lee. Dr. Lee will triage each case. The patient will either be referred to the ER or to drive up to the parking lot by the rear entrance and call our office
upon arrival. For high risk patients, Dr. Lee will then have the patient roll down the window and obtain the vitals and do a lung exam and then afterwards if necessary will get a nasal pharyngeal sample. Low risk patients will enter through the back door into the back exam room with the HEPA filter. 

    3. Front office staff will wipe down all door handles (front/back), and countertops every 90 minutes in the lobby, bathroom, and scale. 

    4. Back office staff will thoroughly disinfect the patient rooms every hour as well as after each patient with respiratory illness leaves the back patient room.

   Our office manager, Tammy Lee, plays a major supportive role in the practice.

   She is always working to make our office a better place, and without out her we wouldn't be where we are today.
In her spare time, Tammy enjoys reading books, long walks, and spending time with her family.

Getting your medical records...

Unfortunately my effort to get the same Electronic Medical Record (EMR) was unsuccessful. Consequently, you will need to get the records the old fashioned way, by paper.

You can drop by the Cascade Park IM office at suite 104 and sign a release of record form to transfer records to:  
 David Lee Internal Medicine 406 SE 131st Ave #109, Vancouver  WA 98683
 you can download it here. After completing the form, you can also fax the form to (360) 254-2894 (if you saw me at Cascade Park Internal Medicine, 406 SE 131st Ave #104, Vancouver  WA 98683).
If you are completely new to our practice then bring in your completed form during your first visit.

Will Dr. Lee accept my insurance?

 On staff since 2011, Kristen began in the health field in 2004, working first in General Practice and Preventive Education. She then decided to go back for her degree in Medical Assisting and subsequently became certified in 2012. "Dr Lee is a wonderful boss -- one of the best I've ever had!" Ask her about her time overseas, or about her latest backpacking adventure.

I am privileged to work with Dr Lee who is so caring, kind and compassionate.
When I am away from work I try to be with my 7 grandchildren as much as I can.

Hanging up my own shingle

The decision to leave Portland Adventist after almost 15 years was a difficult one, and yet I'm hopeful about the future.  My belief is that I'll be able to provide better care as an independent practioner.  I hope to find innovative and viable solutions for those of you who have lost your job and/or your health insurance.  I would also like more time with my patients which a 'group practice' tends to restrict.
