Our new Electronic Medical Record is now up and running. You may now schedule your first appointment after registering your demographic and insurance information!

 

step 1: go to www.patientally.com and select SIGN UP NOW

step 2. You will provide your e-mail address and choose an ID.  A temporary password will be e-mailed to you so you can now LOG IN. You will then need to create your own PASSWORD

step 3. Select MY PROVIDERS on the top menu and click SEARCH. You will find me in the list after selecting WA (state). Then click ADD.  You will also need to enter my SECURITY CODE 147862.

step 4: You can now go to PROFILE on the top tab and select PATIENT PROFILE to upload your Informations (Demographic, Medical History, Medication, Allergy, Preferred pharmacy and Insurance)

step 5: select HEALTH CARE REQUEST on the top tab and select APPOINTMENT to make your first appointment.
